Prior to admission, a senior manager will visit the perspective resident to conduct an assessment aimed at ensuring that we fully understand and are able to meet the individualised care needs of the person. Visits to the home are welcomed and an overnight stay is encouraged to ensure that the perspective resident can experience the high quality care that we offer at Oakland Nursing Home, meet the dedicated team and talk to other residents who already benefit from our warm, homely and safe environment. Once you have decided that Oakland is right for you, on admission you will be allocated a key worker who will ensure that further assessment of your wishes, likes and dislikes are taken into account whilst developing a plan of care that will focus on your holistic needs and have you firmly at the centre of the decisions that will maximise your independence and complement this with high class care where deemed necessary.
You can expect to be included within reviews of your plan of care to ensure that it is updated regularly and evolves to meet any changing needs that are identified.
We are aware that it is important to you that your family and loved ones are able to continue their involvement and regular contact. Therefore, in line with your personal wishes, we will include the people that are closest to you when determining the best care pathway for you. Furthermore, we work in conjunction with GP’s, community health services and other professional care providers to ensure that your care and treatment is professionally supported in meeting your needs.
We are committed to providing the best value for money within our comprehensive and caring service, regardless of your financial status.
